Hello parents and welcome to the 2019-20 band year. All students are required to fill out the band forms packet and submit several checks for uniforms, booster dues and, if applicable, instrument rentals.
Here's the summary...ALL band students must do the following:
1) Complete online Band Forms - ONLINE COMPLETION IS PREFERRED. (Printable PDF is available.)
2) Print and complete the SMSD Transportation Release form.
3) Are you renting a SMSD instrument? Print and complete the Instrument Rental Form.

Now...here are the details...
Forms Please print these forms and bring them with the appropriate checks to Strike Up the Band on Tuesday, July 30th. I will be there to receive them. Hard copy forms and checks can also be brought to the band room and turned in to Mr. Adams during band camp or the first week of school.
Payment
if your student is using funds from their student account, please make note on the forms. We cannot receive payment by credit card at this time since the funds potentially go to three different accounts. Please send all payments by checks payable to the correct group.
