I hope everyone enjoys ANOTHER day off!
Regarding the first payment and form turn in that is due tomorrow for the Europe trip, here are some answers to some questions and some pointers from Mrs. Velicer who is collecting forms and checks.
First, currently, payments need to be made by check. We are not setup to collect through credit card or PAYPAL at this point.
Along with your check, all forms are due-please make sure your read them and fill them out as required including notarized signatures where required.
All travelers also need to open an account with Super Holiday Tours. The instructions on how to do that are included in the packet that you picked up at the meeting.
If you are not a member of symphonic band, do not turn anything in unless you are specifically invited by Mr. Adams after we see the signups from the symphonic band.
PLEASE KEEP READING...here are is a message from Mrs. Velicer.
I hope you had a wonderful Thanksgiving! I am starting to receive trip forms and payments in the mail and there are some common mistakes or omissions showing up.
1. Each parent needs to sign a green form. A few families have had one parent sign both green forms and just write the other parent’s name in the bottom paragraph. My understanding is that each parent needs to separately fill out and sign a green form with the opposite parent’s name written in the space shown in the bottom paragraph.
2. The medical form also needs to be accompanied by a front and back copy of the insurance card. I know some people are getting new cards in January and have let me know they would update that information at that time. So far no one has sent this.
3. Please ask parents to write their child’s name in the memo line on the checks for each payment, it makes my life a lot easier and is a quick way for me to double check. If they are bringing cash, please ask them to put it in an envelope with the student’s name on the front so there is no question as to who it belongs to.
If you need additional forms, I will have them in the band office tomorrow...assuming we finally have school again!
